Masters in Sports Physiotherapy:
(a) Bachelor in Physiotherapy with at least 50% marks from any recognized University/Institute
or equivalent or higher degree – OR (b) M.B.B.S. |

in Sports Physiotherapy (Hons.):
(a) Bachelor in Physiotherapy with at least 50% marks from any recognized University/Institute
or equivalent degree - OR (b) M.B.B.S. |

Bachelor in Physiotherapy:
10+2 in Medical Stream with minimum of 50% marks or any other examination equivalent
thereof. |

Postgraduate Diploma in Hospital Management: MBBS
/ BDS / BPT /B.Sc. Genetics / Nursing / Human Biology / Biotechnology / B. Pharmacy
/ B.E. / B. Tech. / B. Arch. / M. Sc. in Life Sciences. |

is based on Entrance Test to be conducted by the Department. |

| Availability
of Application Forms:
Against payment of Rs. 550/- at the Cash Counter of the University or Rs. 600/-
through post by sending crossed Bank Draft drawn in favour of Registrar, Guru
Nanak Dev University, payable at Amritsar, from Assistant Registrar (General)
Guru Nanak Dev University, Amritsar-143005. Forms can also be downloaded from
the website www.gndu.ac.in. The downloaded forms
must accompany the price of the prospectus and entrance test fee, put together
in form of Bank Draft. |

| Note:
Candidates can submit their application forms without late fees with the normal
entrance test fees of Rs. 650/- for the Postgraduate Courses and Rs. 450/- for
the Undergraduate Course in the form of bank draft favouring of Registrar, Guru
Nanak Dev University, payable at Amritsar, provided the draft is made before or
on 26-06-2009. The application thus submitted will be entertained even if received
after 26-06-2009 without late fee. Candidates can apply after 26-06-2009 with
late fee (as mentioned in the prospectus) till the day before the date of examination.
